This is something I learned at a Google Apps for Education summit, so am not the genius that thought of it, but have made a helpful doc explaining how to do it.
I created a google form for recording learning skills observations. Then added it to my iphone home screen so that I could use voice typing on my phone, rather than go to the computer when I notice something that I want to record.
